Here is my humble opinion: Generally, there is little use for any procedural sedation. Folks that sick or with that level of a problem would most likely be sent on to a referral facility. However, now that we have docss there in Ciriboya, there is no reason not to teach about it. Will be glad to do a talk on precedural sedation. It may well be something they could strart to do. In my expienece, that level of sedation has never been needed (so far).
We have a reputation for not having any narcotics. I would keep it that way. Yes there are incredible problems with the importation of those agents. The biggest problem would be to have it known that we have it so deep in the narco-traffiking lanes. I think it might be a threat to life. Personally I do not ask about such things while there, but am more comfortable treating all citizens with a fair minded equanimity....some things I just don't need to know...
Injectable local anesthetics are greatly needed...lido, lido plus epi. marcaine, and especially large quantities of dental marcaine plus epi.
